09.15.23 From Ali Miller (8): SEEK Trips Info & Waivers

Here is a lot of information about the SEEK trips!

Please fill out the waiver online by Tuesday, 9/19 and take a screenshot once you are done. Please share that screenshot with me.

The waiver for Montreal is one you need to download. It is listed below as MONTREAL ADEX RELEASE form. Please fill it out and return it to me (either on paper or digitally).

Students going on the Burlington trip must also complete the waiver below, LOCAL MOTION BIKE RENTAL WAIVER. Your child can return the paper copy to me by Tuesday or you can scan/take a photo of it and email it back to me.

Students going on the Montreal trip need to get a document notarized. We are very fortunate that an 8th grade parent is a notary and has volunteered to notarize them for us. That form is NOTARIZED PERMISSION TO TRAVEL FORM.  Please return to me by Wednesday, 9/20. The notary will then come to school and notarize them for us. You do not need to be present for that. For this form, both legal guardians are required to complete a notarized copy. If there is only one legal guardian, they will need to provide proof of single guardianship.

Also for Montreal, please send, by Wednesday 9/20, (or before), the identification your child will be using for proof of citizenship. For American citizens, this can be a birth certificate, a passport or a passport ID card. For non-US citizens, it needs to be a passport. We will keep these locked up at school. On the trip, the head chaperone, Ms. Traub, will be in charge of these. 

Also, for Montreal, if the drop-off time on Wednesday, 9/27 is 6:30 am. It will not be possible for them to take the D-E bus that day. (More details on arrival on 9/29 to come.)

For all other trips, your child CAN take the D-E bus. (I gave the Burlington trip group incorrect information today.)
